What is the highest dose of semaglutide you can take? Max doses for Wegovy, Ozempic and pills

The highest dose of semaglutide is 7.2 mg once a week, sold as Wegovy HD. It was approved in March 2026 for people who have taken Wegovy 2.4 mg for at least 4 weeks and need more weight loss. The highest dose depends on which semaglutide brand you take:

  • Wegovy injection: 2.4 mg weekly as a standard dose, or 7.2 mg weekly as Wegovy HD
  • Ozempic: 2 mg weekly
  • Wegovy pill: 25 mg daily
  • Rybelsus: 14 mg daily, or 9 mg in the newer formulation

In trials, Wegovy 2.4 mg led to about 15% weight loss, and 7.2 mg led to about 19% to 21%. Here’s how long it takes to reach each top dose, what to expect and whether you need the highest dose at all.

Key takeaways

  • The highest semaglutide dose is 7.2 mg weekly (Wegovy HD), approved for weight loss in March 2026
  • Wegovy’s standard maximum is 2.4 mg weekly, reached no sooner than week 17
  • Ozempic tops out at 2 mg weekly for type 2 diabetes
  • The Wegovy pill tops out at 25 mg daily. Pill and injection doses aren’t comparable, because much less of a pill is absorbed
  • Digestive side effects are common at every dose. On Wegovy 2.4 mg, 44% of people had nausea vs 16% on placebo
  • You don’t have to reach the highest dose. Wegovy 1.7 mg is also an approved maintenance dose

Highest dose of semaglutide by brand

Semaglutide comes in four brands. Each has its own dose schedule and maximum dose:

Brand Form Approved for Starting dose Maximum dose
Wegovy Weekly injection Weight management, heart risk reduction, MASH 0.25 mg 2.4 mg
Wegovy HD Weekly injection Additional weight loss after 2.4 mg 7.2 mg (after 2.4 mg) 7.2 mg
Wegovy pill Daily tablet Weight management, heart risk reduction 1.5 mg 25 mg
Ozempic Weekly injection Type 2 diabetes, heart and kidney risk reduction 0.25 mg 2 mg
Rybelsus Daily tablet Type 2 diabetes 3 mg (or 1.5 mg in the newer formulation) 14 mg (or 9 mg in the newer formulation)

Never compare doses across forms. A 25 mg Wegovy pill isn’t stronger than a 2.4 mg injection. Only a small share of a semaglutide pill is absorbed, so pills need much higher milligram doses to work.

Wegovy dose schedule: how to reach the highest dose

Wegovy starts low and increases every 4 weeks to reduce side effects:

Weeks Wegovy dose
1 to 4 0.25 mg
5 to 8 0.5 mg
9 to 12 1 mg
13 to 16 1.7 mg
17 onward 2.4 mg (maintenance)
After at least 4 weeks on 2.4 mg 7.2 mg (Wegovy HD), if your provider recommends more weight loss

The 0.25 mg, 0.5 mg and 1 mg doses are steps on the way up. The label lists 1.7 mg and 2.4 mg as maintenance doses. If 2.4 mg causes too many side effects, your provider may lower you to 1.7 mg.

That’s the fastest timeline. If side effects are hard to manage, your provider can delay a dose increase by 4 weeks. Many people take longer, and that’s fine. For timing rules, see how long to stay on each GLP-1 dose before increasing.

Wegovy HD: the 7.2 mg semaglutide dose

Wegovy HD (semaglutide 7.2 mg) is the highest semaglutide dose available. The FDA approved it in March 2026 for adults with obesity who:

  • Have taken Wegovy 2.4 mg for at least 4 weeks
  • Tolerated 2.4 mg well
  • Need additional weight loss, as judged by their provider

In the STEP UP trial, adults with obesity and without diabetes took 7.2 mg, 2.4 mg or placebo for 72 weeks:

Result at 72 weeks Semaglutide 7.2 mg Semaglutide 2.4 mg Placebo
Average weight loss About 19% About 16% About 4%

On 7.2 mg, nearly half of participants lost at least 20% of their body weight, and about 1 in 3 lost at least 25%. In people who stayed on treatment, average weight loss on 7.2 mg was about 21%. In a separate trial of adults with obesity and type 2 diabetes, 7.2 mg led to about 13% weight loss vs about 10% on 2.4 mg.

The most common side effects at 7.2 mg were nausea, diarrhea and tingling or altered skin sensation. Most were mild to moderate and eased over time. Wegovy HD costs $399 a month through NovoCare self-pay.

Highest dose of Ozempic

Ozempic is approved for type 2 diabetes, and its dose is guided by blood sugar:

Weeks Ozempic dose
1 to 4 0.25 mg (starter dose)
5 onward 0.5 mg
After at least 4 weeks on 0.5 mg, if needed 1 mg
After at least 4 weeks on 1 mg, if needed 2 mg (maximum)

Ozempic 2 mg is the highest diabetes dose. It’s lower than Wegovy’s 2.4 mg, so Ozempic usually leads to less weight loss. Ozempic isn’t approved for weight loss. If weight loss is your main goal, Wegovy is the approved option.

Highest dose of the Wegovy pill and Rybelsus

Wegovy pill: You start at 1.5 mg once daily and step up about every 30 days, from 4 mg to 9 mg and then to the 25 mg maintenance dose. In the OASIS 4 trial, the 25 mg pill led to about 14% to 17% weight loss over 64 weeks. You need to take it on an empty stomach with up to 4 ounces of water and wait at least 30 minutes before eating.

Rybelsus: The original tablets go from 3 mg to 7 mg, and then to 14 mg if more blood sugar control is needed. A newer formulation uses 1.5 mg, 4 mg and 9 mg tablets. The two aren’t interchangeable milligram for milligram, so never mix them.

Results at the highest dose of semaglutide

Here’s what the main Wegovy trials found at 2.4 mg:

Trial What it tested Result
STEP 1 (68 weeks) Wegovy 2.4 mg vs placebo 14.9% vs 2.4% average weight loss
STEP 4 (weeks 20 to 68) Staying on 2.4 mg vs switching to placebo A further 7.9% loss vs 6.9% regain
STEP 4 (overall, 68 weeks) Continuous 2.4 mg 17.4% total weight loss
SELECT Wegovy 2.4 mg in people with heart disease 20% lower risk of heart attack, stroke or heart-related death

STEP 4 shows why staying on treatment matters. People who switched to placebo regained much of their weight. For how long people stay on the top dose, see how long to stay on the maintenance dose of Wegovy.

Side effects at the highest semaglutide doses

Digestive side effects are common with semaglutide. They’re most likely in the days after a dose increase and usually ease over time. Here’s what the Wegovy label reports at 2.4 mg:

Side effect Wegovy 2.4 mg Placebo
Nausea 44% 16%
Diarrhea 30% 16%
Vomiting 24% 6%
Constipation 24% 11%
Abdominal pain 20% 10%
Fatigue 11% 5%

Rapid weight loss can also lead to hair shedding and facial volume loss, sometimes called “Ozempic face”. Rare but serious risks include pancreatitis, gallbladder problems and kidney injury from dehydration. Get medical care right away for severe stomach pain that spreads to your back, or vomiting you can’t stop.

If tiredness is the problem, see why some people feel extreme fatigue on Wegovy.

When is your semaglutide dose too high?

Your dose may be too high, or increasing too fast, if you have:

  • Nausea or vomiting that lasts more than 1 to 2 weeks after an increase
  • Trouble eating enough protein or drinking enough fluids
  • Dizziness, weakness or signs of dehydration
  • Weight loss that’s faster than is healthy

Your provider may keep you on your current dose longer, lower it or step you back to 1.7 mg. For more warning signs, see how to know when your GLP-1 dose is too high.

Semaglutide overdose: how to avoid it

Taking more than your prescribed dose can cause severe nausea, vomiting, dehydration and low blood sugar. Most reported overdoses involve compounded semaglutide from multi-dose vials. There, people measure doses with a syringe and can easily confuse milligrams with units. Some people took 5 to 20 times their intended dose this way.

To stay safe:

  • Use FDA-approved pens or pills, which deliver a fixed dose
  • Never double up after a missed dose
  • Never take more than the maximum dose for your brand

If you think you’ve taken too much, contact your provider or Poison Control at 1-800-222-1222. Learn more in what happens if you overdose on Ozempic.

Do you need the highest dose of semaglutide?

No. The best dose is the lowest one that helps you reach your goals with manageable side effects. Some people do well on 1.7 mg and stay there. Others reach 2.4 mg and stop there, without ever needing Wegovy HD.

Consider staying at your current dose if:

  • You’re losing weight steadily
  • Side effects are still settling
  • You’ve reached your goal and are moving into maintenance

Consider moving up if:

  • Your weight loss has stalled for several weeks despite good habits
  • Hunger is returning
  • Your provider thinks more weight loss would benefit your health

See what happens if you don’t increase your semaglutide dose for what to expect at a lower dose. Some people also ask about very low doses to limit side effects. Our guide to GLP-1 microdosing explains what the evidence shows.

What if the highest dose of semaglutide isn’t enough?

If you’ve plateaued on 2.4 mg or 7.2 mg:

  1. Check the basics: protein, liquid calories, alcohol, sleep and missed doses
  2. Add strength training to protect muscle and support your metabolism
  3. Get labs to rule out thyroid problems or other causes
  4. Talk about switching. Tirzepatide (Zepbound) led to more weight loss than semaglutide in a head-to-head trial. Its own maximum dose is 15 mg. See the highest dose of Zepbound

The bottom line

The highest dose of semaglutide is 7.2 mg weekly (Wegovy HD), available to people who’ve tolerated Wegovy 2.4 mg for at least 4 weeks. Wegovy’s standard maximum is 2.4 mg, reached no sooner than week 17. Ozempic tops out at 2 mg, the Wegovy pill at 25 mg daily and Rybelsus at 14 mg, or 9 mg in the newer formulation. Higher doses lead to more weight loss, but they also bring more digestive side effects, and many people do well on lower doses. Never change your dose on your own. Make each step with your provider.
