In 2026, semaglutide costs anywhere from $0 to more than $1,300 a month. Where you land depends on the brand, the dose and how you pay. Without insurance, most people pay $149 to $499 a month through the manufacturer’s self-pay programs. That’s far below the list prices of about $1,000 for Ozempic and $1,349 for Wegovy. With commercial insurance that covers it, a savings card can bring the copay down to as little as $25. Medicare patients may qualify for $50 a month through a new pilot program. Below you’ll find a price table for every semaglutide brand and the 7 main ways to pay less.
Key takeaways
- Semaglutide is sold as Wegovy (pens and pills) for weight loss, and as Ozempic and Rybelsus for type 2 diabetes
- Self-pay prices through NovoCare Pharmacy: Wegovy pens $349 a month, Wegovy HD $399, Wegovy pills $149 to $299
- New Wegovy patients can pay $199 a month for their first 2 fills, through December 31, 2026
- With commercial insurance, the Wegovy savings offer can cut your cost to as little as $25 a month (maximum savings $100 a month)
- Since July 2026, eligible Medicare patients can get Wegovy for $50 a month through the GLP-1 Bridge program
- Novo Nordisk’s patient assistance program provides Ozempic free to some uninsured people with low incomes
- There’s no FDA-approved generic semaglutide in the US, and compounded versions are not FDA approved
Semaglutide brands and what each one is for
Semaglutide is the active ingredient in several brands made by Novo Nordisk. They work the same way, but they’re approved for different uses and priced differently.
| Brand | Form | FDA-approved for | Approved since |
|---|---|---|---|
| Wegovy | Weekly injection, 0.25 to 2.4 mg | Chronic weight management, heart risk reduction, MASH | 2021 |
| Wegovy HD | Weekly injection, 7.2 mg | Additional weight loss after 2.4 mg | 2026 |
| Wegovy pill | Daily tablet, 1.5 to 25 mg | Chronic weight management, heart risk reduction | 2025 |
| Ozempic | Weekly injection, 0.25 to 2 mg | Type 2 diabetes, heart and kidney risk reduction | 2017 |
| Rybelsus | Daily tablet | Type 2 diabetes | 2019 |
This matters for cost. Insurance plans and savings programs treat weight loss and diabetes prescriptions differently. Wegovy is the brand that’s approved for weight loss, so it’s the one most weight loss savings programs apply to.

Two things stand out. First, the self-pay price for Wegovy pens is the same at every dose up to 2.4 mg, so raising your dose doesn’t raise your cost. Second, the Wegovy pill gets more expensive as your dose goes up. That’s the opposite of how the pens work.

What semaglutide costs per year
Most people stay on semaglutide for a year or longer, so it helps to think in yearly terms:
| Option | First year | Each year after |
|---|---|---|
| Wegovy pens, self-pay with intro offer | About $3,888 | About $4,188 |
| Wegovy pill, self-pay, working up to 9 or 25 mg | About $3,338 | About $3,588 |
| Wegovy HD, self-pay | About $4,788 | About $4,788 |
| Ozempic 2 mg, self-pay | About $5,988 | About $5,988 |
| Wegovy at list price | About $16,188 | About $16,188 |
| Wegovy with Medicare GLP-1 Bridge | $600 | $600 (program runs through 2027) |
These are estimates based on monthly prices. Your total depends on your dose, how long you take it and any price changes.
Treatment length matters for cost. Many people regain weight after stopping, so semaglutide works best as a long-term treatment. We explain the timeline in how long you take Wegovy for weight loss.

Does insurance cover semaglutide for weight loss?
It depends on your plan and why you need it.
- For type 2 diabetes: Most commercial and Medicare drug plans cover Ozempic or Rybelsus, usually with prior authorization.
- For weight loss: Coverage for Wegovy varies widely. Many employer plans exclude weight loss drugs, and others require prior authorization, a minimum BMI or proof that you’ve tried diet and exercise first.
- For heart risk reduction or MASH: Some plans that exclude weight loss drugs will cover Wegovy for these approved uses.
- Medicare: Part D plans don’t cover drugs for weight loss alone. Some cover Wegovy for heart risk reduction or MASH, or Zepbound for sleep apnea. For others, the GLP-1 Bridge program is a new option (see below).
- Medicaid: Coverage depends on your state.
If your plan says no, ask your provider about a prior authorization or an appeal. Learn more in what to do if Wegovy isn’t covered by insurance.
Where to find discounts on semaglutide: 7 ways to pay less
1. Use the Wegovy savings offer if you have commercial insurance
If your commercial plan covers Wegovy, the Wegovy savings offer can lower your copay to as little as $25 a month. The maximum savings is $100 a month, so you may pay more if your copay is high. People with Medicare, Medicaid or other government insurance can’t use it with their coverage. Ozempic and Rybelsus have similar savings cards for people with type 2 diabetes and commercial coverage.
2. Buy through NovoCare Pharmacy at the self-pay price
If you have no coverage, Novo Nordisk’s self-pay program is usually the cheapest legal way to get brand-name semaglutide. You pay $349 a month for Wegovy pens, or $199 for your first 2 fills as a new patient through December 31, 2026. Pills start at $149 a month. You can use NovoCare Pharmacy home delivery or bring the savings offer to a participating local pharmacy.
3. Check the Medicare GLP-1 Bridge program
Since July 1, 2026, Medicare patients with Part D drug coverage can get Wegovy pens or pills for a $50 monthly copay if they qualify. The pilot runs through December 31, 2027. Eligibility includes:
- A BMI of 35 or higher, or
- A BMI of 30 or higher with conditions such as stage 3a or higher chronic kidney disease, certain types of heart failure or uncontrolled high blood pressure, or
- A BMI of 27 or higher with prediabetes, a previous heart attack or stroke, or blocked arteries in the legs or arms
Your provider submits the request. People already covered for a GLP-1 under Part D, such as for type 2 diabetes, aren’t eligible for the Bridge.
4. Apply for the Novo Nordisk patient assistance program
Novo Nordisk’s patient assistance program (PAP) provides some medicines free to people who qualify. In 2026, uninsured people with income at or below 200% of the federal poverty level can get Ozempic at no cost. Medicare patients with drug coverage are no longer eligible for Ozempic through the PAP. The program does not list Wegovy, so check NovoCare’s current product list before you apply.
5. Compare telehealth and retail options
Several telehealth platforms and retail programs now offer brand-name semaglutide at the manufacturer’s self-pay price. Compare the full cost, including visit fees and memberships, not just the medication price. At Heally, a free doctor consultation is included, Wegovy injections start at $199 a month and Wegovy pills start at $149 a month, with no membership fee.
6. Use your HSA or FSA
Prescription weight loss medications are often eligible for health savings account (HSA) or flexible spending account (FSA) funds when they treat a medical condition such as obesity. Paying with pre-tax money can save you a significant amount over a year. Check your plan’s rules.
7. Choose the form and dose that fit your budget
Your choice of product can change your monthly cost by hundreds of dollars:
- Wegovy pens cost the same at every standard dose, which helps if you need 2.4 mg
- Wegovy pills are cheapest at lower doses, starting at $149
- Ozempic 2 mg is the most expensive self-pay option at $499 a month
For the pros and cons of each form beyond price, see Wegovy pills vs injection.
What about compounded or generic semaglutide?
There is no FDA-approved generic semaglutide in the US. The main US patents aren’t expected to expire for several years.
Compounded semaglutide became popular during the 2022 to 2024 shortage. The FDA declared the shortage over in February 2025, so pharmacies can no longer mass-produce copies. Compounded versions are not FDA approved and aren’t reviewed for safety, effectiveness or quality. The FDA has received reports of dosing errors and hospitalizations linked to them. Some products also use semaglutide salt forms, which the FDA says are different from the approved drug.
With brand-name Wegovy now available from $149 to $349 a month, the price gap is much smaller than it used to be. Watch out for sites selling semaglutide without a prescription. They’re a common source of counterfeit products.
How semaglutide cost compares with other weight loss medications
| Medication | Typical self-pay price per month | Average weight loss in key trials |
|---|---|---|
| Wegovy (semaglutide 2.4 mg) | $349 | About 15% |
| Wegovy pill (semaglutide 25 mg) | $299 | About 14% |
| Zepbound (tirzepatide) | From $299 through LillyDirect | About 20% |
| Liraglutide, generic | Varies by pharmacy | About 7% to 8% |
| Metformin | Often under $20 | Modest, usually a few pounds; not FDA approved for weight loss |
Weight loss figures are averages from separate clinical trials, so compare them with caution.
Tirzepatide now costs about the same as semaglutide at self-pay prices and led to more weight loss in a head-to-head trial. Semaglutide has its own strengths, including pill options and proven heart protection. For a detailed comparison, see tirzepatide vs semaglutide: dosage and results.
Hidden costs to plan for
The medication isn’t the only expense. Budget for:
- Provider visits. Some programs charge for the first visit, follow-ups or a monthly membership
- Lab tests. Your provider may check blood sugar, kidney function and other markers before you start. A weight loss lab panel covers the basics
- Supplies. Sharps containers for pens, and sometimes pen needles for Ozempic
- Side effect support. Over-the-counter remedies for nausea or constipation in the first weeks
- Missed savings. Letting a savings card lapse or missing a refill can mean paying full price
How to start semaglutide affordably
- Check your insurance first. Call your plan or check its formulary for Wegovy and ask whether prior authorization is needed
- If you’re covered, activate the savings offer to bring your copay down
- If you’re not covered, compare self-pay options, including NovoCare Pharmacy and telehealth programs
- If you have Medicare, ask your provider about the GLP-1 Bridge program
- If your income is low and you’re uninsured, check the patient assistance program
- Talk to a provider about which form and dose fit your health needs and budget
The bottom line
Semaglutide is far more affordable in 2026 than its list price suggests. Without insurance, most people pay $149 to $499 a month through Novo Nordisk’s self-pay program, with Wegovy pens at $349 and a $199 intro price for the first 2 fills through the end of 2026. With commercial insurance, the savings offer can bring the cost down to $25 a month. Eligible Medicare patients can now pay $50 a month. Avoid unapproved compounded or no-prescription products, and compare the full cost of any program, including visits and memberships.

Does Medicare cover semaglutide for weight loss?
Part D doesn’t cover drugs for weight loss alone. However, since July 2026, eligible Medicare patients with Part D coverage can get Wegovy for $50 a month through the GLP-1 Bridge program, which runs through 2027.
Is there a generic version of semaglutide?
No. There’s no FDA-approved generic semaglutide in the US. Compounded semaglutide is not FDA approved and can no longer be mass-produced since the shortage ended in 2025.
Does the dose affect the cost of semaglutide?
For Wegovy pens, no: the self-pay price is $349 at every dose from 0.25 to 2.4 mg. For Wegovy pills and Ozempic, yes: higher doses cost more. Wegovy HD (7.2 mg) costs $399 a month.
