Is Wegovy covered by insurance? 10 alternatives if your Wegovy prescription isn’t covered

wegovy no insurance

Wegovy is covered by some insurance plans, usually with prior authorization, but many plans still exclude weight loss medications. If yours doesn’t cover it, you have more options in 2026 than ever. Without coverage, you can buy Wegovy pens directly from Novo Nordisk for 349amonth(199 for your first 2 fills through December 31, 2026). The Wegovy pill starts at $149 a month. Eligible Medicare patients can get Wegovy for $50 a month through the GLP-1 Bridge program. You can also ask whether your plan covers Wegovy for heart or liver disease, or whether it covers another medication such as Zepbound or Foundayo. Here are 10 alternatives, ranked roughly from the cheapest to the most work.

Key takeaways

  • Many plans don’t cover Wegovy for weight loss, and almost all require prior authorization if they do
  • Some plans that exclude weight loss drugs will cover Wegovy for its other approved uses: heart risk reduction and MASH liver disease
  • Self-pay prices: Wegovy pens $349 a month, Wegovy HD $399, Wegovy pills $149 to $299
  • Eligible Medicare Part D patients can pay $50 a month for Wegovy through the GLP-1 Bridge, through December 2027
  • Covered alternatives may include Zepbound, Foundayo, Ozempic (for type 2 diabetes) or older weight loss medications
  • Never switch to unapproved compounded semaglutide or online sellers without a prescription

Is Wegovy covered by insurance?

Wegovy is the brand of semaglutide approved for weight management. Whether your plan covers it depends on:

  • Your plan type. Employer, marketplace, Medicare and Medicaid plans all have different rules
  • Your employer’s choices. Many employers decide whether to include weight loss drugs at all
  • Your diagnosis. A plan may refuse Wegovy for weight loss but cover it for heart disease or MASH
  • Your health details. Most plans require a BMI of 30 or higher, or 27 or higher with a weight-related condition, plus records of past weight loss efforts

Under the Affordable Care Act, plans must cover obesity screening and counseling, but not weight loss drugs.

In 2026, Wegovy is a preferred weight loss drug on many large formularies, including CVS Caremark’s. Still, your employer must opt in, and prior authorization is usually required.

How to check your Wegovy coverage

  1. Search your plan’s formulary for Wegovy, and note any prior authorization (PA) or step therapy (ST) requirements
  2. Call the number on your insurance card and ask whether Wegovy is covered for weight management, heart risk reduction or MASH
  3. Ask HR whether your employer excludes weight loss medications

The prior authorization and appeal process works the same way for every GLP-1. We walk through it step by step in will my insurance cover Zepbound.

10 alternatives if Wegovy isn’t covered by insurance

1. Appeal, or ask about Wegovy’s other approved uses

A denial isn’t always final. If it was about missing records, your provider can appeal with your BMI history, weight-related conditions and past weight loss efforts.

Wegovy is also FDA approved for two uses beyond weight loss:

  • Heart risk reduction in adults with obesity or overweight and established heart disease, such as a past heart attack or stroke. In the SELECT trial, Wegovy lowered the risk of heart attack, stroke and heart-related death by 20%
  • MASH (formerly NASH) with moderate to advanced liver fibrosis, approved in 2025

Some plans that exclude weight loss drugs, including many Medicare Part D plans, cover Wegovy for these conditions. If one applies to you, make sure your provider includes it in the prior authorization.

2. Use the Wegovy savings offer

If you have commercial insurance, the Wegovy savings offer can lower your cost to as little as $25 a month when your plan covers Wegovy. The maximum savings is $100 a month. If your plan doesn’t cover Wegovy, the same offer gives you access to self-pay prices at participating pharmacies. It can’t be used with Medicare, Medicaid or other government coverage.

3. Pay the self-pay price through NovoCare Pharmacy

For most people without coverage, this is the simplest option:

Wegovy product Self-pay price per month
Wegovy pen, 0.25 to 2.4 mg 349(199 for the first 2 fills for new patients, through December 31, 2026)
Wegovy HD pen, 7.2 mg $399
Wegovy pill, 1.5 mg $149
Wegovy pill, 4 mg $199
Wegovy pill, 9 mg or 25 mg $299

That’s far below Wegovy’s list price of about $1,349 a month. For first-year totals and price traps to avoid, see what you’ll pay for Wegovy with no insurance.

4. Switch to the Wegovy pill

The Wegovy pill contains the same medicine as the injection, taken once a day. It’s the cheapest way to start Wegovy without insurance, at $149 a month for the starting dose. In the OASIS 4 trial, people on the 25 mg pill lost about 14% to 17% of their body weight over 64 weeks. You need to take it on an empty stomach with a little water and wait 30 minutes before eating. Compare both forms in Wegovy pills vs injection.

5. Use the Medicare GLP-1 Bridge

Medicare Part D doesn’t cover drugs for weight loss alone. Since July 1, 2026, the GLP-1 Bridge pilot lets eligible Medicare patients get Wegovy pens or pills for a $50 monthly copay, through December 31, 2027. You generally need one of the following:

  • A BMI of 35 or higher
  • A BMI of 30 or higher with stage 3a or higher kidney disease, certain types of heart failure or uncontrolled high blood pressure
  • A BMI of 27 or higher with prediabetes, a previous heart attack or stroke, or blocked arteries in the legs or arms

6. Ask if your plan covers Zepbound

Some plans cover Zepbound but not Wegovy, or prefer one over the other. Zepbound contains tirzepatide and led to more weight loss than semaglutide in the SURMOUNT-5 head-to-head trial: 20.2% vs 13.7% over 72 weeks. It’s also approved for obstructive sleep apnea, which some plans, including Medicare Part D, will cover. Without coverage, LillyDirect sells Zepbound from $299 a month. See tirzepatide vs semaglutide for a full comparison.

7. Try Foundayo, a newer GLP-1 pill

Foundayo (orforglipron) is Lilly’s daily GLP-1 pill, approved for weight loss in April 2026. Unlike the Wegovy pill, it can be taken with or without food and water. In its main trial, people on the highest dose lost about 11% to 12% of their body weight. Self-pay prices start at $149 a month for the lowest dose. With commercial coverage, a savings card can bring the cost to as little as $25 a month. Foundayo is also part of the Medicare GLP-1 Bridge.

8. Ask about Ozempic if you have type 2 diabetes

Ozempic contains the same drug as Wegovy but is approved for type 2 diabetes. If you have diabetes, your plan is much more likely to cover Ozempic, and it can also help with weight loss. Plans won’t cover Ozempic for weight loss alone, and using it that way is off-label. Without coverage, Ozempic’s self-pay price is $349 a month for doses up to 1 mg.

9. Consider an older, lower-cost medication

If newer GLP-1s are out of reach, older options may be covered or affordable:

Medication Typical average weight loss Notes
Liraglutide (Saxenda and generics) About 5% to 8% Daily GLP-1 injection. Generic versions became available in 2025
Phentermine About 3% to 7% Generic pill, often about $10 to $50 a month with discount coupons. Approved for short-term use
Phentermine and topiramate (Qsymia) About 7% to 10% Generic versions can cost far less than the brand
Naltrexone and bupropion (Contrave) About 5% to 6% Daily pill. Not for people with seizures or uncontrolled high blood pressure
Metformin Small, a few pounds Diabetes drug, sometimes used off-label for insulin resistance

Weight loss figures are averages from separate clinical trials and vary by person.

These medications usually lead to less weight loss than Wegovy, but they can still help, especially with diet and activity changes. For how liraglutide compares in practice, see how long liraglutide takes to work.

10. Use HSA or FSA funds, or change plans

  • HSA or FSA: Prescription weight loss medications are often eligible when they treat obesity. Paying with pre-tax money lowers your real cost
  • Open enrollment: If you’re choosing a new plan, check each formulary for Wegovy and its prior authorization rules before you enroll. Compare the full cost, including premiums and deductibles
  • Medicare open enrollment runs from October 15 to December 7 each year

What about the patient assistance program?

Novo Nordisk’s patient assistance program provides some medicines free to people with low incomes. In 2026, it doesn’t list Wegovy. Uninsured people with income at or below 200% of the federal poverty level may qualify for free Ozempic if they have type 2 diabetes. Check NovoCare’s current product list before you apply.

Alternatives to avoid

Some “cheap Wegovy” options carry real risks:

  • Compounded semaglutide. It’s not FDA approved, and the FDA has received reports of dosing errors and hospital stays linked to it. Since the semaglutide shortage ended in February 2025, pharmacies can no longer mass-produce copies
  • Online sellers without a prescription. These are a common source of counterfeit pens
  • Splitting or stretching doses to make pens last longer. Single-dose pens aren’t designed for partial doses
  • Using someone else’s prescription. It’s unsafe and illegal

With Wegovy pills from $149 and pens at $349 a month, the savings from these risky options are much smaller than they used to be.

How to choose the right alternative

Your situation Best place to start
Commercial plan covers Wegovy Savings offer, as low as $25 a month
Commercial plan doesn’t cover Wegovy Self-pay through NovoCare, or the Wegovy pill from $149
Medicare with Part D GLP-1 Bridge for $50 a month, or coverage for heart disease, MASH or sleep apnea
Type 2 diabetes Ozempic or Mounjaro coverage
Plan covers Zepbound instead Zepbound with a $25 savings card
Tight budget Wegovy or Foundayo pill at $149, or an older medication

The bottom line

If your insurance doesn’t cover Wegovy, start by checking whether you qualify through a heart or liver condition, and appeal if the denial was about paperwork. Without coverage, Wegovy costs $349 a month through NovoCare, or $199 for your first 2 fills through 2026, and the Wegovy pill starts at $149. Medicare patients may pay $50 a month through the GLP-1 Bridge. Zepbound, Foundayo, Ozempic for diabetes and older medications are other options. Avoid compounded or no-prescription products, and choose the option that fits your coverage, health and budget with a provider’s help.

FAQ

Why isn’t Wegovy covered by my insurance?

Many plans exclude weight loss medications, and the Affordable Care Act doesn’t require them to cover these drugs. Other common reasons for denial are missing documentation, not meeting BMI criteria or step therapy rules.

How much is Wegovy without insurance?

Through NovoCare Pharmacy, Wegovy pens cost $349 a month at every dose up to 2.4 mg, or $199 for the first 2 fills for new patients through December 31, 2026. Wegovy pills cost $149 to $299 a month, and Wegovy HD costs $399.

What is the cheapest alternative to Wegovy?

For eligible Medicare patients, the GLP-1 Bridge at $50 a month. With commercial coverage, the savings offer at $25 a month. Without coverage, the Wegovy pill or Foundayo at $149 a month for the starting dose. Older medications like generic phentermine cost less but lead to less weight loss.

Does Medicare cover Wegovy?

Not for weight loss alone. Part D plans can cover Wegovy for heart risk reduction in people with heart disease, or for MASH. Since July 2026, eligible Medicare patients can also get Wegovy for $50 a month through the GLP-1 Bridge.

Is Zepbound a good alternative to Wegovy?

For many people, yes. Zepbound led to more weight loss than semaglutide in a head-to-head trial, and some plans cover it when they don’t cover Wegovy. Your provider can help you decide based on your health and coverage.

Can I get Ozempic instead of Wegovy?

If you have type 2 diabetes, your plan may cover Ozempic, which contains the same drug. Plans won’t cover Ozempic for weight loss alone.

Is compounded semaglutide a safe alternative?

It’s not FDA approved, and the FDA has received reports of dosing errors and hospital stays linked to compounded GLP-1s. With brand-name Wegovy now available from $149 to $349 a month, it’s usually better to use an approved option.
